If your patient is not satisfied with either their Bausch + Lomb ULTRA® ONE DAY, Bausch + Lomb ULTRA® or Biotrue® ONEday contact lenses, you can return them for credit providing the product:
● Has a minimum of 12 months shelf life
● Is returned within 60 days from date of invoice
● Is returned together with the original paperwork/order details
Please fill in all details on the returns form below. Upon submission of this form, you will receive a reference code and a postage label to print off for the return of the product.
Please enclose the reference code with the returned product.
Please download our pre-paid address label to attach to your return pack.
